4. SUMMARY OF PROBLEM - DURING THE INVESTIGATION OF A CH-47
ACCIDENT IN ALASKA, THE PROBABLE CAUSE WAS ISOLATED TO THE SUSPECTED
MATERIAL FAILURE OF A SCREW IN THE UPPER BOOST ACTUATOR CONTROL
VALVE. THIS FAILURE WOULD RESTRICT THE TRAVEL OF THE PILOT VALVE IN
THE UPPER BOOST ACTUATOR RESULTING IN REDUCED CONTROLABILITY OF THE
