             66-19094, Boeing build number B-352, was a CH-47A helicopter. The U.S. Army acceptance date was 4 May 1967. The administrative strike date was on or about 7 April 1972. 66-19094 accumulated at least 2,315.0 aircraft hours prior to transfer to the Vietnamese Air Force (VNAF).

   At some point, 66-19094 was assigned to the 237th Helicopter Squadron, Vietnamese Air Force (VNAF), located in the Republic of Vietnam (RVN), and remained there until it was lost due to enemy fire.

   66-19093 received small arms fire from the ground and was forced to land. A fire broke out and consumed the aircraft. There were no reported casualities.

   The last known location of 66-19094 was in the Republic of Vietnam.
