             91-00269, Boeing D model kit number M3418, was a CH-47D helicopter. The U.S. Army acceptance date was 22 October 1992. As of 22 June 1998, 91-00269 had accumulated 520.0 D model hours and 3,591.3 total aircraft hours.

   91-00269 was a conversion from the original A model Chinook 61-02421.

   On 1 November 1991, 91-00269 was inducted into the D model program, converted, and scheduled for assignment to the Army National Guard, located in the State of Nevada.

   At some pont, 91-00269 was transferred to the detachment located in Pendleton, Oregon.

   At some point, 91-00269 deployed to Afghanistan to conduct operation in support of the "War on Terrorism".

   On 4 December 2005, while conducting a troop transport mission in the combat environment, 91-00269 was struck by small arms fire which severed hydraulic lines. The aircraft lost hydraulic power at 15 feet above ground level (AGL) and caught fire.

   There were 34 personnel aboard, consisting of the crew and passengers. All managed to evacuate the aircaft but had to escape and evade (E & E) during the ensuing ground fight. Two soldiers were shot, but not seriously. The aircraft was consumed in the post-crash fire.

   As of 4 December 2005, this aircraft was 42.4 years old.

   As of 4 December 2005, the last known location of 91-00269 was approximately 50 miles north of Kandahar, Afghanistan, assigned to Company D, 113th Aviation, Army National Guard, 6th Army.

   Aircraft status: Shot down in combat.
